11/21/2023 EnchantaVaughn Approved B: QMHP arrived to the home for individual session with client and mother. Mother reports client has been having issues not respecting those who are in authority on a daily basis. When redirected by her mother, she will storm out the room lashing out on others with for 30 minutes in duration. B: Client self-reported to QMHP on how client does not solve conflict with others in the home as she yells as screams at her elderly grandparents 6-9 times daily lasting 3-5 minutes in duration. I: QMHP began the session by inquiring about clientâs current thoughts, feelings, and emotions. QMHP engaged client in conversation regarding her recent behaviors, progressions, and setbacks. QMHP was able to get client to communicate with her family in a calm tone, by issuing rules before talking. QMHP reminded client that the rules consist of allowing each family member to speak without interruption. QMHP engaged client on the importance of exploring conflict. QMHP educated the client during the session that conflict involves having a disagreement with someone. I: QMHP shared with the client that no matter how hard we try to avoid a conflict they are sure to keep happening. QMHP explained that it is possible to learn how to resolve conflicts in such a way that everyone is happy. QMHP processed with the client and shared some positive ways to resolve conflicts, such as peer mediating, talking to an adult about the situation, using I feel statements to express how you feel so you do not offend the other person. QMHP challenged client to explain how he would resolve a conflict in a positive manner using one of the methods discussed or using another positive method. R: Client admitted to counselor that she does find it difficult with adapting to the new rules and regulations in the home. Client understands that her family is trying to aid her in prioritizing skills and create structure in the home. Client mentioned that she has noticed her family has been making efforts on breaking the communication barrier in the home. Client also verbalized that her mother has also been consistent with her activeness in communicating with her and supporting her during this new transition. R: Client stated that her mother has been praising and encouraging her when she displays positive and respectful behaviors. Client was able to reduce physical overactivity and become more attentive in the home. Client implemented appropriate social interaction skills. Client displayed good study and organizational skills with her therapy work. Client displayed an understanding of ADHD and its symptoms and causes. Client reflected on what it means to fully receive help and support others in the home. Client displayed healthy self-esteem. Client smiled and said thank you. Client was observed and reported eliminating personal conflicts at home.
